We are now looking for a nurse/community nurse who wants to contribute to our operations and provide the best possible care and service to our residents. As a nurse/community nurse with us, you will be part of an engaged and competent team where we individually and collectively take responsibility. In the small municipality, we take advantage of our closeness to each other, which creates good opportunities for collaboration with other professions. In the nursing unit, there are colleagues with different experiences and competencies, which means we complement each other well. We support each other, share knowledge, and help each other in challenging situations with the patient's best interest in focus.

RESPONSIBILITIES

As a nurse/community nurse, you will work with home care both in regular housing and in special accommodations. You are responsible for planning and executing care interventions based on individual needs and the patient's best interest while collaborating with other professions.

Your role includes, among other things:

  • Planning, assessing, implementing, and evaluating nursing care.
  • Assessing patients' health status through both patients' subjective experiences and objective data.
  • Working preventively by identifying, noticing, and communicating risks, both for the individual and in the organization, and actively contributing to systematic patient safety work.
  • Noticing and acting appropriately on changed situations and conditions based on your nursing responsibility.
  • Performing advanced care.
  • Providing support and advice to other professionals around the patient.
  • Maintaining good contact with relatives and creating security for the patient.
  • Performing documentation and other administration related to the patient's care.
  • Collaborating with other professions.

As a staff member working evenings and weekends, you have a leadership role for nursing staff working in regular and special accommodations.

QUALIFICATIONS

  • Licensed nurse, preferably with specialist training as a community nurse, but this is not a requirement.
  • Experience from municipal health and medical care is meritorious but not a requirement.
  • Independent, responsible, and possess good communication skills. Good Swedish in speech and writing is of great importance.
  • Have a good approach and the ability to create trusting relationships with both patients, relatives, and colleagues.
  • Have a positive attitude and an interest in change and development within healthcare.
  • Driver's license (B) is required.

With your ability to plan, organize, and prioritize, you contribute to safe and effective care. You are attentive and trustworthy in your interactions with patients.
